e-Forms

Generation stage

The eForms are available on the site of the Education, Audiovisual, and Culture Executive Agency.
More information is available in the eForm User Guide .

Reinvestment stage

Reinvestment applications:

Until the eForms are available, applicants must use the following paper forms:

Module 2

Annexes:

• All modules : Declaration on the applicant's honour  
• Module 2 : The estimated budget

The application package must be sent by registered mail or private courier no later than the submission deadline to the following address:

Annexes

The following documents must be filled in and attached to the e-form

Generation stage

• Declaration on the applicant's honour
• The list of films and international sales form
• Description of the project
• Financial Capacity Form

Reinvestment stage

• All modules : Declaration on the applicant's honour  
• Module 2 : The estimated budget

Application package

Compulsory annexes that cannot be submitted online have to be sent to the Agency by post:

Generation Stage

For each film that has not been submitted to the Agency as part of a previous application:

• The financing plan of the film
• One copy of the international sales contract

The application package must be sent by registered mail or private courier no later than the submission deadline to the following address:

A cover letter must indicate:

• the name of the company
• PIC number
• the E-form confirmation number
